STEP 1
To register a loved one as a donor with Aeternitas Life, the authorizing person must review and complete an authorization for anatomical donation and cremation.
If you are pre-registering yourself as a donor, you must list yourself as the authorizing person.
The authorization for anatomical donation and cremation will authorize the anatomical donation and cremation of the donor, including the postmortem release of the donor’s medical records.
STEP 2
The authorizing person must provide the names and contact information for two witnesses.
One witness can be a genetic relative. The second witness can be a legal relative, friend, colleague, or hospice representative.
The witnesses will receive a copy of the authorization for anatomical donation and cremation in order to validate the authorization for anatomical donation and cremation.
STEP 3
If you are working with a hospice provider, please provide the name and contact information for the hospice representative.
The hospice representative or hospice organization will be responsible for the postmortem release of the donor’s medical records to Aeternitas Life.
The hospice representative will receive a copy of the completed authorization for anatomical donation and cremation to confirm your approval.
