Congratulations on taking steps toward completing college coursework while in high school!  This is the Early College Credit Program application form for Wisconsin Private Colleges.  Each form can hold four (4) course requests for one (1) institution.  You will need to use a separate form for each private college you want to request courses for, or if you have more than four course requests at one institution.

Please complete the form in its entirety, including checkboxes.  After a student has submitted this application, it will automatically go to the parent/guardian and counselor you list below for their review and approval.  Please make sure to double check name spellings and email addresses before initiating this application.

You will receive school district approval once all course requests are presented to and approved by the school board at its March 19th meeting.
