Congratulations on taking steps toward completing college coursework while in high school!  This is the Start College Now application form for courses held at MATC.  Each form can hold eight (8) course requests.  You will need to initiate another form after this one has been completed if you have more than eight courses.

Please complete the form in its entirety, including checkboxes.  After a student has submitted this application, it will automatically go to the parent/guardian you list below for review and signature.  Please make sure to double check name spellings and email addresses before initiating this application.

You will receive school district approval once all course requests are presented to and approved by the school board at its October meeting.
